Annual Co-Payment You pay an annual co-payment before the plan pays for exams, frames, or lenses. Once you pay the annual co-payment, all covered services will be covered at 100% up to the benefit maximums. Under the Vision II plan, you must pay an additional co-payment before receiving a 2nd pair of frames or lenses.
